The UGA School of Medicine is committed to fostering a safe, supportive, and professional environment in all interactions with colleagues, students, patients, and their families. In accordance with the UGA SOM Professionalism Policy and Student Mistreatment and Learning Environment Policy, this form provides a manner to report concerns of professionalism, student mistreatment, or other conduct affecting the learning environment.

Excused Absence Request Form
Any misrepresentation of reason for absence request is a violation of the UGA SOM Honor Code of Academic Integrity signed by all students. Any absence that is not approved as an excused absence is by definition an unexcused absence and considered a breach of professionalism. Violations of Professionalism and/or the Honor Code of Academic Integrity are subject to review by the Academic and Professionalism Progress Team (APPT) and potentially Student Promotions Committee (SPC).
